EYE CATALOG AND ORDER FORM

Below is a low resolution digital catalog for customers to use when preparing orders. DMC eyes are created in higher resolution than what you see in the catalog. 

To request custom "MADE TO ORDER" eyes simply use the Eye ID's from the catalog and enter them into the Order Form below. 

Please read the instructions thoroughly and then fill out all fields before you click submit.  

 After you submit a request for custom eyes from this catalog, I will reach out to confirm I got your order within 5-7 days. Estimated turnaround time for completion of your order varies but typically runs 8 weeks. As soon as I complete your order, I'll send an invoice to the email address you provided when ordering.
